LIABILITY WAIVER REGARDING TRANSPORTATION POLICY

I understand and acknowledge that by allowing myself or my child to ride in a car with an affiliate of Ho'aka Mana, I am engaging in a potentially risky activity. I fully understand and accept the risks associated with riding in a car, including but not limited to accidents, injury, or death. In consideration of being permitted to participate in this activity, I hereby release and forever discharge Ho'aka Mana, its partners, contractors, and volunteers from any and

all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury that may occur as a result of me or my child riding in a car with an affiliate of Ho'aka Mana. I fully understand and acknowledge that I am assuming all risks associated with this activity and that I release Ho'aka Mana, its partners, contractors, and volunteers from any and all liability, including negligence, that may arise from my participation. I have carefully read and fully understand the terms of this liability waiver and voluntarily sign the relating registration/rsvp form.
